What does one entry in your Reading error notes look like? The phrase you missed, why the wrong answer was tempting, or both?
Also how do you check it helped on a new passage? Repeating one you remember doesn't seem a fair test.

Does ONE WORD ONLY tell us anything about singular or plural? Or do we need the full question and, for Listening, the audio too?

ONE WORD ONLY just tells you the number of words. The answer could be singular or plural. The passage or audio decides that, so check the grammar around the gap too.
